    At Optum, your health is our top priority. Living with health problems and managing them can be hard. That's why we offer a variety of care management programs to be with you every step of the way.


    Care Management complements and enriches the care already provided by your primary care provider, specialists, and other health care providers. You may benefit from Care Management if you have any of the following:


    • Go to the hospital more often than you would like to
    • See several health care providers for your condition(s)
    • Take multiple medications
    • Have questions about your condition(s) or your treatment plan(s)
    • Find it difficult to stick to the lifestyle changes your provider has recommended

     


    Optum Care Management Services offers a variety of interventions that can help you manage your condition(s) and improve your overall quality of life. When you participate in a Care Management program, you’re connected with a team of dedicated health care professionals — nurses, social workers, and others — who will give you personal, one-on-one assistance.

     

    • Our goal is to help remove any barriers that may get in the way of you better managing your health.
    • We make sure you understand your condition(s), your treatment options, and your provider’s treatment plan(s).
    • We provide resources to help you make informed decisions about your health care.
    • We guide you in methods to improve how you manage your condition(s) day-to-day.
    • We work with the providers on your health care team to assist in coordinating your needs.
    • We help you understand your benefits.
    • We connect you to community resources that offer additional support.

     


    We work on your schedule — how often you talk to us is up to you. Our program is of no cost to you. Care Management is a benefit of your health plan. When you participate in Care Management, you’ll be assigned to a Care Manager — a registered nurse — who will advocate for you as you negotiate the sometimes complicated health care system. Your Care Manager will make sure you receive the best care at the right time, and in the most appropriate place. Your Care Manager will work with you, your caregivers, and your health care providers to create an individualized plan of care to help you better understand and better manage your condition(s). Your Care Manager will continue to advocate for you until your condition is improved, or you’ve reached a point where you can easily manage your own care.


    There are many ways to get started with our Care Management program. One way is to let your Optum provider know that you would like a referral, or you may self-refer by calling us at 1-505-232-1600 (TTY:711) Monday through Friday from 8 am to 5pm. Again, this program is offered at no cost to you. Participation in any Optum program is optional, and you may choose to leave the program at any time.
